INTRODUCING High Mountain Health's Patient Portal

A new way to securely and conveniently access your health information and communicate with High Mountain Health.

Our new Patient Portal makes it easy to access your health information and provides you the ability to conveniently:

  • Request medication refills (for non-controlled substances)
  • Request a referral
  • Direct questions to our clinical staff
  • Direct questions to our billing staff
  • Receive lab, x-ray and study results
  • Review your medical history, add updates,make changes, verify your medication list and allergies
  • Print a continuity of care record with your medical information, which you can take to other appointments
  • Access your records from your specialists' offices, allowing your other doctors to view, print or upload the information from your chart to their electronic records

 

Registering for the Patient Portal is simple. Fill out the Patient Portal Authorization Form and bring, scan or fax it back to us. Once you've registered, visit the Log In page by clicking the button below (or use the button from our homepage), enter your User Name and Password and that's it. It's that simple.
